Lubasha Visnjic: Ïîëíûå ñïîéëåðû íà àíãëèéñêîì. From Mrs Vhttp://mrsviw.blogspot.com/ ************************************************************* Teaser: Sam tells Luka that she was going to tell him (about moving out). She says she doesn’t want to hurt him, but she needs time alone to think things out. She grabs her coat and purse. Luka asks her where she’s going, and she says she’s looking for a new apartment on her way to work. Luka says, “So that’s it, we don’t talk about this?” She replies, “Never seems to do us any good”. She leaves. Cut to Abby running to catch the El train. Two businesswomen named Drew and Erika hear her asking them to hold the door, but they don’t. The doors close, but Abby manages to jam her hand in the door, causing it to open again. As she gets on, she sarcastically thanks them for holding the door for her. We see Neela’s on board, and she says, “Good morning”. Seems it isn’t a good morning for Abby – she was up all night finishing a paper on paint ball injuries. Neela guesses that the paper’s for Morris. Abby wants to know if Morris said anything about it. Neela says no, and mentions that Morris has her doing a paper on adolescent asthma. Abby guesses that Morris is going to be listed as the first author. She’s right. They hear the 2 women laughing – they seem to be whispering about Abby and Neela. Abby says to them, “Can I help you?” One woman asks where Abby got her shoes. The second woman says they look comfortable. The women pretend to be sincere about what they’re saying while Abby and Neela look down at their shoes. One of the women says that the shoes match the backpack. Neela and Abby look at each other – it’s hard to be fashionable when they’re hardly getting any sleep and are up to their armpits in body fluids. Abby tells Neela to ignore them – they’re “obviously on a bulimia high”. Neela can’t believe how rude those women were to Abby. Abby says they were rude to Neela, too, since she’s the one with the backpack. Neela defends her choice, saying she can’t put her books in a handbag. All 4 get out, with Abby and Neela trailing behind. One of the rude women catches her heel in something and falls down. Neela thinks they should make sure she’s OK, but Abby doesn’t seem to have any intention of helping them. Neela goes back to check. The woman tells her that she can’t stand, so Neela tells her not to move. Neela grabs her ankle, and the woman tells Neela not to touch her. Neela explains that she’s a doctor. The women share a look and say they want a second opinion. Neela turns to Abby and says, “Dr. Lockhart, would you care to give another opinion?” Abby replies, “Sure—your ankle’s broken and you’re a bitch. And you might want to consider some new shoes”. End of teaser.

Lubasha Visnjic: Somewhere in Act 1: I only have the end of Luka and Jerry’s conversation. Luka says, “Yeah, why?” Jerry replies, “Think he might have died”. As Luka moves to take the phone call, Morris sidles up to the board with some interns. He tells them that he’s going to show them how he likes things done. He sees Neela, Abby and Ray and tells them that what he’s about to say applies to them, too, because he’ll be implementing some changes. Neela says, “I’d like to stick an implement in his eye”. Ray asks her to pick a number from 1 to 10. Neela says 7. Abby says 4. At this point, Sam enters with a pregnant woman on a gurney. The woman, Claire, is 24 years old and 9 months pregnant. She’s described as an earth mother. Ray tells Neela and Abby they were both close – the number was six. He tells Neela that she gets to choose between 2 interns: Munson and Babinski. Abby tells them to wait a sec, but Neela goes ahead and chooses Babinski. We get the bullet on Claire: she was rear-ended at moderate velocity. She wasn’t wearing her seatbelt and now complains of abdominal pain. Sam wants to know which one of them will take the patient. Ray will and asks Jane to join him: “I trust you, girl. Don’t let me down”. Jane breaks away from Morris’s tutorial to join Ray. Luka seems to be stuck on the phone as Sam heads off with Ray and Jane. Sam asks Claire why she wasn’t wearing a seatbelt, and Claire says she thought it would harm her baby. Ray tells her that she thought wrong. Claire’s having contractions and is breathing right through them, controlled. Sam, who’s being assisted by a new, young nurse named Inez, is looking at the fetal monitor and tells them that Claire’s definitely having contractions. Ray examines Claire and tells her that he doesn’t think she’s bleeding internally. Inez notes that Claire’s underwear is wet, and Jane mentions that it’s premature rupture of the membranes. Claire asks if that’s bad. Ray tells her that it isn’t, but she’ll probably give birth today. She says she wanted a home birth, and Ray tells her that he thinks it’s best if she stays in the ER. We find out that Claire will be giving birth to her second child. Sam asks if Claire wants her to call her hubby, but Claire says she doesn’t have one. She should call her friend, Toni, instead. Luka enters and gets the bullet from Ray. He gives out a few orders, then asks Sam if she has a minute. She says she doesn’t and lists all the things she has to do. Luka looks at Inez, wondering if she can help Sam. Sam mentions that she’s helping Inez “get the lay of the land”. He asks if she can see him when she’s finished. She agrees and mentions that it might be awhile before she can do that. He says he’ll be there and heads out. Ray can sense the tension between Luka and Sam and tries to hide it from the newbies. Ray welcomes Inez to County, and she thanks him. Then Ray addresses Claire, telling her that they’ll move her upstairs (to OB?) before she can get comfortable. She doesn’t think she’ll be comfortable for several more hours. Ray and Jane step away. Ray asks Jane what she’ll do next, and she guesses that she should do a bimanual exam. He tells her that she shouldn’t do it before the ultrasound. She wants to know why. He explains that they should rule out previa before they do an exam. He also says that the most important thing she should remember is that if she doesn’t get it, fake it. There’s no time to ask a lot of questions and patients don’t like to see her scratching her head. Ray asks her to get some Nitrazine paper. She doesn’t know what that is, but Ray gestures to her to get going, so she does. Jane doesn’t know what she’s looking for and doesn’t seem to be able to find it in any of the carts in the hallway, so she goes to the drug lockup. Abby enters to get some meds, and Jane asks her about the Nitrazine paper. Abby says, “You didn’t ask him?” Jane says, “He said if I didn’t know something…” Abby interrupts and tells her that the #1 rule is that if she doesn’t know something, she should ask. That’s what they’re there for. She’s not going to be able to learn this stuff on her own. Abby gives her the paper and moves away before Jane can ask her another question.

Lubasha Visnjic: Jane’s in the hallway now and sees Neela with Babinski. She asks Neela if she could tell her what she’s supposed to do with the paper. Neela say she can’t help right now, and also tells her that if she doesn’t know how to do something…Jane interrupts with, “I know, fake it”. Neela says, “What? Oh God, no. Of course not.” She explains that if Jane doesn’t know something, she should look it up herself. If someone gives her the answer, she’ll forget it 5 minutes later. If she reads about it, the info’ll stick. Her primary responsibility is to educate herself because no one’s going to do it for her. Neela leaves, and Jane’s just standing there, still not knowing what to do with the paper. Cut to Abby with Munson, looking at an x-ray. Abby tells him her name, but he says he already knows. A note says, “This dude is really odd”. Abby asks him if he has any questions. He says no. She thinks that’s great and asks him if he’s ready to present. He says, “I’ll let you know”. Abby says, “Okay…Can I make a suggestion?” 2 pages later, Chuny asks Pratt if he can get on the MICN – some paramedics need him to pronounce a GSW in the field. He says sure. As he moves off with Chuny, Abby turns back to Munson, who is having a hard time changing the battery on his otoscope. Malik enters with a patient for Abby. The paramedic gives her the bullet: the patient, whose name they don’t know, is about 10 and was found unconscious at a mall. He’s hypotensive despite being given treatment in the field, and his mental status was “waxing and waning” en route. Abby tells Munson to go with her. They walk up to the gurney, and Abby introduces herself to the kid. The kid says something, but Abby can’t make it out. Abby asks the paramedic if the kid was alone. The paramedic says that he was with his friends, but they left when the paramedics arrived. Munson notes that the kid’s sats are low. Abby asks him what he thinks he should do next. He says a few things, and she asks if there’s anything else he should do. After the third time she asks him about his treatment plan, he guesses that he should call next of kin. Abby lists all the tests that need to be taken. Munson feels dejected. Abby tells him that he’ll get the hang of it. He says that he knows. A note says, “Off Abby—this guy is a dick”. Cut to Pratt talking to the paramedics on the MICN as Luka does paperwork nearby.

Lubasha Visnjic: 2 pages later and we’re in Act 2. Ray and Jane are examining Claire. Ray says he doesn’t see evidence of placental abruption. Pratt enters. A note says that even though he’s affected by seeing his father, he tries to shake it off. Claire wants to know if Ray just gave her some good news. He says that he did, and explains that trauma can sometimes cause the placenta to separate from the uterus. This didn’t happen in her case. Ray starts to introduce Pratt but Pratt interrupts, telling Ray that he’s going to need a trauma room soon. He asks how they’re doing. Claire says she doesn’t know about him, but she’s having contractions. Jane holds up a forcep with a paper strip and declares that nitrazine is positive. Sam explains that that means her water did break. Ray and Pratt look at the screen – they’re very concerned. Claire asks what’s going on, and Ray tells her that her baby is in breech position, which could complicate matters. Claire’s concerned, so Pratt tells her that OB will give them their best advice. Jane calls Ray and gestures for him to look at the speculum. Claire mentions that her pregnancy is no different from her last one. The baby’s active, though. Sam gives her some words of encouragement. A note says that whatever Ray saw in the speculum catches him off-guard. He asks Pratt to take a look. Claire asks if there’s anything wrong. Pratt looks into the speculum and we switch to… Speculum cam. Two little feet are coming out of the cervix. Ray tells Claire that it looks like she delivered the baby’s feet. Claire asks if that’s OK, then tells them that she had a friend who delivered breech and the baby came out just fine. Pratt tells someone to call OB and get them down to the ER. Sam tells Claire not to worry – they’re going to take care of her and the baby. Claire tells Sam that it’s not really her baby. Cut to Chuny escorting Toni over to Luka. Toni says that someone called and said that Claire was in an accident. Luka tells her that Claire’s uninjured. Toni’s relieved, then asks about the baby. Luka asks if she’s a friend of the family. Toni explains that Claire’s a “gestational surrogate”. She’s been living with Toni and her husband, Rob, for the last 6 weeks. Luka asks if Claire’s having Toni’s child. Toni says yes and explains that they had IVF and the embryo was implanted in Claire. Toni asks again about the baby, and Luka says that he can’t tell her without Claire’s consent. Toni argues that she’s the intended parent of the baby, but Luka won’t tell her; he says sorry. Toni says that Claire and her are best friends – Claire would want to see her. As they near the room, Sam walks out to grab some supplies. Luka introduces Toni to Sam. Sam says they spoke on the phone and tells her to go in – Claire’s been asking for her. Sam holds the door open for Toni. Luka asks how Claire’s doing, and Sam says she’s good. He gives her a look, so she says, “When I get a chance, OK?” He says yeah.

Lubasha Visnjic: Sam re-enters the room, and we see that Claire and Toni are genuinely happy to see each other. Toni says she was worried, and Claire says she’s fine. Toni tells Claire that she told her that she shouldn’t drive; she offered to drive Claire around. Malik enters, looking for some charcoal. He sees Pratt and tells him there’s a guy in the waiting room looking for him. Pratt snaps at Malik and says he knows. Claire bears down through another contraction, and Ray and Sam react. Toni asks if she’s OK, and she nods. Toni asks Ray if she’s in labor, and Ray says yes. Toni asks if they’re going to stop the contractions because the baby isn’t due for another week. Ray explains that at 39 weeks, the baby is more than ready to come out. Sam mentions that the baby’s feet are already sticking out. Toni knows that the baby’s turned the wrong way. Claire says the baby’s fine. Toni asks Ray if they’re going to do a C-section. Ray looks at Pratt for an answer. Sam jumps in and says it may not be necessary. Ray says they’re waiting for OB. Pratt tells Ray to find him once OB arrives, then tells Inez to follow him. Inez doesn’t know what to do, so Sam tells her to go – she’ll stay with Claire. Inez leaves. Claire congratulates Toni at this point and tells her she’s going to be a Mom today. Pratt walks to Admit. Neela (with intern in tow) is about to ask him if he can help them but he interrupts and tells her to find Morris instead. Pratt leaves in search of Charlie. Neela turns to her intern and gives him a sympathetic look. She assures him that he made a good catch, then asks him to present the next patient. He tells her about a patient with a calcaneal fracture and warns her that the patient screamed at him about the amount of time she’s had to wait. Neela takes the chart and heads over to the patient. It’s Drew, the woman from the El! The woman says, “Oh, God. Can this day get any worse?” Neela forces a smile. Cut to Pratt putting on a trauma gown as he walks through the waiting room. Inez is with him. Charlie stands when he sees Pratt and calls him by his first name. Pratt says he doesn’t have time to talk right now. He keeps walking, so Charlie follows him. Charlie says it looks like he’s busy. Pratt insists he is and asks Charlie what he wants. Charlie says he doesn’t know – what was Pratt looking for when he came looking for him? Pratt says he was looking for nothing. He guesses he just wanted to see for himself. Cut to the three of them in the ambulance bay. A note says neither Charlie nor Pratt know the rules to this game. Charlie says, “So how do we do this?” Pratt tells him that they don’t and he’s sorry he bothered Charlie.

Lubasha Visnjic: Pages later, Ray enters Claire’s room to find Coburn working on her. Claire says she doesn’t understand why they can’t just flip the baby around from the outside. Coburn explains that it doesn’t work after her water breaks. Sam adds that it also won’t work because she’s already delivered the baby’s knees. Claire asks for Toni, and Ray tells her that Toni’s calling her husband on the phone. Coburn asks Ray if he found Pratt yet, and Ray says that Pratt’s working on another patient next door. Coburn says that Claire’s going to need a C-section. Claire says she doesn’t want it – her friend had a breech birth. Ray explains that her friend probably had a Frank Breech, where the baby’s butt gets delivered first. Coburn tells Claire that she has a footling breech. If the cervix doesn’t dilate enough, the baby’s head will get stuck and the umbilical cord will get compressed with each contraction, jeopardizing oxygen supply. Ray tells Claire that she needs to do what’s best for the baby. Claire asks Coburn if she’s ever done a delivery like this before. Coburn says she has, but the baby was smaller. Claire asks what will happen if she doesn’t have the surgery. Coburn says they’ll allow the baby to be delivered to the umbilicus, which will give the cervix time to dilate. Then they’ll “use maneuvers” to deliver the head and shoulders. Claire wants to do that, but Ray tells her it’s not a good idea. Coburn explains that there’s a 25% risk of major complications with a footling breech and only 2% with a C-section. Claire says that there’s a 75% chance that the baby will be born w/o any problems. Coburn looks at Sam and Ray, then tells Claire they’ll do it her way. They’ll see if the labor proceeds. But if the baby’s heart rate drops too low, they may still have to do a C-section. Claire asks for the baby’s heart rate and is told that it’s 164. Claire says that everything seems fine – she doesn’t want the surgery. As Coburn leaves, she tells them that she’ll send somebody down as soon as a room’s available upstairs. Ray and Sam share a look. Claire asks for Toni, so Ray says he’ll see if she’s off the phone. However, Ray’s real intention for leaving the room is to talk with Coburn. Ray asks, “Are you really not going to consent for a C-section?” Coburn says she won’t do it against Claire’s will. Ray asks her what she’ll do if there’s any trouble with the baby and a C-section needs to be performed. She says that she can’t do it. Ray asks if Coburn’s kidding. Coburn says that Claire has the right to decide. Ray says, “What about Toni? It’s her baby too.” 2 pages later, Abby tells Jerry she’s still waiting for test results. She’s hoping PICU’s on the phone, calling to admit the 10 yo. Why is he asking? Jerry says that his buddies are asking about him. Abby sees the kids through the triage window and tells Jerry to keep the kids there. Jerry moves off to do that, and Abby gets on the phone. It’s the PICU, and it seems they’re ready to take the 10 yo. She tells them that they’re still waiting on his labs but they’ll send him up. She hangs up and tells Malik to prep the kid for the PICU.

Lubasha Visnjic: Abby walks to the waiting room. The kids (one of them is carrying an extra backpack) are standing near Jerry. She tells them that she needs to know what their friend took. The kids start running, but Jerry catches one of them – the one carrying the extra backpack. His name’s Dermott. He wants Jerry to let go, but Jerry won’t. Abby tells Dermott that his friend’s really sick and she needs to know his name. Dermott says his friend’s name is Liam. Abby asks Dermott if Liam took anything. He didn’t. She wants to know why they ran when the paramedics arrived. He says it’s because they skipped school and that all they wanted to do was get some lunch at the mall. Abby asks if Liam’s been sick. Dermott doesn’t think so, but Liam hasn’t been in school for awhile. Abby asks why not and is told that his parents are getting a divorce and he has to shuttle back and forth between his parents. That’s why they skipped school – they just wanted to hang out together. Abby asks if Liam has any medical conditions. The only thing Dermott knows is that Liam’s parents are vegetarians. A note says that Abby’s trying her best not to lose her patience as she asks him if Liam was acting differently today. Dermott says he might’ve been bummed – he ate 2 jumbo cheeseburgers. Abby says she thought Dermott said that Liam’s a vegetarian. Dermott just shrugs. Munson calls for Abby and tells her that Liam’s getting worse. Abby asks Dermott if he has Liam’s backpack. He does. She says she needs it. She also wants Dermott to give Liam’s name and phone number to Jerry STAT. Abby walks back inside the room and tosses the backpack to Munson. 3 pages later, Abby mentions to Munson that Liam ate 2 cheeseburgers. Munson doesn’t know understand. Malik doesn’t either and tells Abby that they need to send Liam upstairs. Abby disagrees – she thinks they need to get his ammonia level. She tells Munson to run and get some ice because ammonia’s got to go to the lab cold. Munson leaves. Malik tells Abby that they can do that upstairs, but Abby says they’ll do it after they run other tests first. Meanwhile, the kid’s brain could be “pickling in”. As Malik leaves, Abby tells Liam to hang in there.

Lubasha Visnjic: Cut to Jane putting an internal ECG monitor on Claire’s baby so that they can get a more accurate read of its heart rate. Jane has her hand in Claire’s vagina, and Ray talks Jane through the procedure. The heart rate’s good, and Claire says she told them she was fine. Ray tells her that that’s only for the time being. If the baby gets into trouble, every second counts when it’s oxygen-deprived. Sam says that that isn’t happening. Ray adds that it’s not happening yet. Claire wants to know why Ray’s trying to scare her. He says he’s not; he’s just trying to prep her for what may happen. He also tells her that thousands of C-sections are performed every day and it’s a simple procedure. Claire says she wants to have more kids of her own one day – naturally. Ray tells her lots of women have natural births after having C-sections. A note says that Claire seems to be softening on the idea. She asks if she can consent, but only if it’s a last resort. Ray says yes, and Sam gives her a form to sign. Sam tells Claire about the risks associated with C-sections, and Ray tries to interrupt. Sam keeps going, saying that the procedure could result in death, and Ray says, “Sam!” Claire says, “Death?” Ray tells her that it’s a minimal risk. Then he asks to speak to Sam outside. Claire hands the form back to Sam and says she won’t sign it. Ray takes the form from Sam as she heads out and tells her to think about. He leaves, and Jane’s the only one left with Claire now. Cut to Ray and Sam at the drug lockup. Pages later, Abby asks Ray if he’s ever done one before (don’t know what they’re referring to). Ray says, “How hard can it be?” He asks for the location of the patient, but she says she’ll ask Pratt to show her instead. Neela tells her that she might want to ask Luka because Pratt’s in a nasty mood. She says Pratt can’t be any worse than Munson. Abby wants to switch interns with Neela in the next shift because Munson’s too weird and he doesn’t like her. Ray says, “Can you blame him?” Abby doesn’t know what he means. Ray clues her in to the fact that Munson was Jake’s roomie. She wants to know why he didn’t tell her that. He says he thought she knew. A note says that Abby “looks like she could kill as Jane comes up”. She tells Ray that Claire needs him. Ray wants to know if the baby’s in distress. Jane says that it’s a domestic dispute. As they leave, Babinski walks up to Neela, “tail between his legs”. BTW, he keeps calling her Dr. Rasgorta. She tells him to just call her Neela instead. He tells her that Drew’s freaking out about how long she’s had to wait. Neela asks Abby to help her put on an ankle cast. Abby sounds surprised that Neela’s asking help on a simple procedure, so Neela explains that the patient’s a friend of Abby’s. Heh. They walk into Drew’s room, and Drew starts complaining. She wants to know if there’s anyone there who can put on her cast. Neela says that she and Abby can do that. Drew says, “Then what the hell are you waiting for?” She urgently needs to attend a conference. Abby apologizes for making Drew wait, but sometimes they lose track of time when they’re saving lives. Drew asks Abby if she’s being sarcastic. She’s the customer in the ER, and if she’s not satisfied, she’ll speak to their supervisor. A note says the scene ends off Abby and Neela: “as they share a look which suggests only more trouble”.

Lubasha Visnjic: Cut to Toni, Rob, and Claire. Rob tells Claire that they had a contract. She says she didn’t agree to surgery. Rob reminds her that she consented to all medical procedures related to the pregnancy. Ray enters and wants to know what’s going on. Rob says Claire needs a C-section and he has a legal document giving them permission to do it. Ray doesn’t think so and asks them to talk with him outside. Rob tells Claire that if she doesn’t do it, she’ll be in breach of contract and she won’t get paid. Claire says she just wants to deliver naturally. Outside of the room, Rob says he’s going to get a court order to force Claire to have a C-section. Ray says he can’t do it, but Rob insists he can. Ray says they’re doing everything they can to convince her, but they can’t force her. Toni mentions how hard and how expensive it was for them to go through IVF. They gave Claire the last of their embryos. Ray says this isn’t the way they should approach the situation. They should let him try again. Sam calls for Ray and tells him that the baby’s heart rate dropped. Toni wants to know what that means. Ray says they have to rush Claire up to OB STAT and tells them that he wants them to wait outside. He goes back into the room where Jane , Sam and Inez are prepping to move Claire. Ray asks Claire how she’s feeling, but she has a hard time talking through the contraction. She asks if she’s being moved. Ray tells her that she is because the baby’s heart rate dropped after her last contraction, which may indicate cord compression. She asks for the heart rate and is told that it’s 112. Claire thinks that that’s still good. Toni and Rob follow the gurney. Rob tries to tell Claire that she really needs the C-section, but Claire argues that she knows her own body and they should trust her. She promises that if anything goes wrong with the baby, she’ll have another one for them. They’re about to wheel Claire into the elevator when Toni says they want to come. Claire says no because they’re stressing her out. Sam tells Toni and Rob to let them get Claire settled, and she’ll be back for them in a few minutes. Cut to Pratt at the board in Admit. A note says he seems to be making no progress. Morris is at the computer with a bunch of paperwork. Pratt asks him if he’s going to treat any patients today. Morris says he’d love to but he’s got a ton of paperwork. Pratt wants to know what he’s doing. Morris says he’s doing chart reviews, teaching conference schedules…CR stuff. Morris says he envies Pratt being able to work “in the trenches” – all he is is a paper pusher. He tells Pratt to stop by his office for a drink – some guys in “utilization review” gave him a good bottle of Macallan’s. Chuny approaches and introduces Karen Simms. We’re told Pratt treated her mother. Pratt doesn’t remember her, so Chuny says, “GSW”. He remembers and thanks Chuny. Karen wants to know what that means, so Pratt tells her. She’s shocked. Pratt tries to explain what happened, but Karen interrupts and asks if her Mom’s dead. Pratt explains what they did to treat her mother and then tells her that the injuries were too severe. Karen wants to know if her Mom was in pain. Pratt says no. She asks if her Mom said anything. Pratt says she was talking a little when she came in, but she became unconscious when she lost a lot of blood. Would Karen like to see her? Karen says it’s too late. Pratt says that she can still say goodbye. Karen replies that her Mom will never know. She says it’s OK – her Mom and her had a falling out years ago and they were both waiting for the other to give in and say they were wrong. She says, “I guess I win. Thanks”. She leaves.

Lubasha Visnjic: Cut to Coburn trying to deliver Claire’s baby. A nurse and Ray are also in attendance. The contraction eases, and we find out that the baby’s hips are out now. Claire can push during the next contraction. Claire tells Ray that she told him she could do it. Ray says, “Yes you did”. Coburn tries to wrap a towel around Claire’s hips for gentle traction but an alarm sounds – the baby’s heart rate is down to 100. And dropping fast. Claire asks if the baby’s OK, and Ray tells her that the baby’s heart rate is dangerously low. The cord’s getting compressed and the baby’s not getting enough oxygen. Ray tells her that this is what they warned her about. Coburn thinks the heart rate will increase between contractions. Claire looks scared. Ray tells her that it’s not too late for her to change her mind and have a C-section. She’ll have a better chance of having a healthy baby. Claire doesn’t want it because the baby’s almost out. Ray says they can push it back in and do the C-section in 2 minutes. Claire thinks she can deliver the baby in 2 minutes. He says they don’t know that for sure – the head could be trapped. Another contraction hits, and Ray tries to convince Claire to have the surgery. Coburn wants Ray to stop. Claire says she can do it. Ray tells her she needs the surgery. Coburn wants Ray to leave Claire alone so that she can concentrate on pushing. Ray tries to convince her again, and Coburn warns him that he’s out if he does that one more time. The act ends with Claire pushing, the alarms going off, and Ray hoping for the best. End of Act 3. Act 4: They’re all doing their best to get the baby out. Claire says she’s really trying. Ray suggests using forceps. Coburn tells Ray to sling the baby around the hips with a towel. Then she uses the forceps on the baby’s head. Coburn asks Claire to push one more time. The baby’s out, and Moog, the pediatrician, says the baby looks cyanotic. They cut the cord and put him in the baby warmer. The baby’s not moving, so they do CPR. Claire thinks Toni and Rob should be allowed to enter and see the baby, but Coburn tells her she wants them to work on the baby first. Claire asks, “Is it bad?” Ray tells her that the baby’s not breathing and he doesn’t have a pulse. “I’d call that bad”. Coburn reprimands him. Ray says that if Claire were really that worried, she would’ve had the C-section. She’d be holding the baby right now if she did that instead of having them shove a tube down his throat. Coburn throws him out. Ray bumps into Toni and Rob in the hallway. They can tell by the look on Ray’s face that it didn’t go well. They ask if the baby’s OK, and Ray tells them that it was a very hard delivery. They want to know if they can see the baby, but Ray tells them they can’t because he’s still being worked on. Toni says, “Working on him?” Ray tells them that the baby was deprived of oxygen. She wants to know what that means. He says they’ll know more once Moog fully assesses the baby. However, there’s a good chance the baby suffered neurologic damage. We see that Toni’s upset and Rob’s angry. Rob wants to go home. Toni says “What about our son?” Ray tells them that Moog will have answers for them soon. Rob says that Ray already gave them the answer. They didn’t pay to have a brain dead baby. Ray says he’s not brain dead. Rob counters that he’s not normal but he would’ve been if Claire had consented to the C-section. Rob walks away. Toni stays and tells Ray that they were going to name the baby after her father. Ray tells her that they still can. Rob says, “Let’s go”. She walks away with him.

Lubasha Visnjic: Cut to Pratt in a high school hallway, looking at a trophy display. He looks at a team photo, and Chas Pratt is amongst those represented. He moves down the corridor towards the swimming pool where a meet’s taking place. He sees Charlie, his wife, Evelyn, and their daughter, Joy, in the bleachers. Pratt’s half-brother, Chas, is about to get on the blocks for the 200 meter freestyle. The Pratt family cheer Chas on. A note says that it seems they’re a very tight family unit, something that is a foreign concept to Pratt. He tries to make sure they don’t see him. Cut to Abby and Liam, who’s getting better. Dermott’s there, too. Abby asks Liam how he feels. He doesn’t feel too good. She tells him that his parents are on their way and that they were worried about him. He says good. Abby advises him to talk with his parents if he’s upset with them. He says OK. Then he says, “My friend thinks your [sic] a MILF.” She doesn’t know what it means, so he says, “Mother-I’d-Like-To”. Abby says she’s not a mother – she’s a doctor. Liam says that he guesses that makes her a DILF. She says goodbye and reminds him to stay on his diet and wear his Medic Alert bracelet. A note says Liam’s wheeled off and Abby wipes his name off the board. “Jake’s postcard is still taunting her. She makes sure no one is watching, then takes it down. She reads the brief message on the back then tears it up and tosses it in the trash under the desk.” She looks up and sees that Neela has been watching her. Neela wants to know if she’s supposed to pretend that she didn’t see that. Abby says, “See what?” Neela replies, “Okay”. A note says, “Abby isn’t going to talk about this—yeah, right”. Abby says she hasn’t heard from Jake but he sent a postcard to the ER. Neela says she practically chased him out of town with a torch. Morris comes up and they both ask him to sign off on their charts. Morris asks about Pratt, and Abby says he left. 2 pages later, it seems Morris has only signed off on one of Neela’s charts. He turns to Abby and says, “Let’s see if you can do better than one for four, Lockhart”. She doesn’t give him her charts. He asks if she needs him to sign off on them. She says no. He thought she said that she had charts for him to sign. She says only Neela said that. He points to her charts and asks if those are hers. She says no. Jane enters and asks someone to show her how to do 2 things. Neela asks about Ray’s whereabouts, and Jane informs her that he never came back down from labor and delivery. Abby and Neela look at each other. Then Neela says he’s probably hitting on the new OB nurses. Abby agrees to teach Jane the 2 procedures.

Lubasha Visnjic: 4 pages later, Pratt says he has to leave. As he goes, Charlie, Evelyn, and Joy approach. Charlie tells Chas he’s hungry. A note says that Charlie’s mood changes the moment he sees Pratt. It’s one of those ‘yikes’ moments. Evelyn notices Charlie’s mood change, so he tries to tell her what happened. She’s surprised and p*ssed, but puts on a good front for Pratt and the kids. Charlie introduces them to Pratt, telling them that Pratt’s his son and he’s a doctor at County. Evelyn thinks it’s wonderful. Pratt thanks her. Charlie invites Pratt out to dinner with them. Chas said he already offered, but Pratt has to work. Maybe some other time. Charlie thinks that would be good. Pratt leaves. “Charlie’s got some ‘splaining to do”. Cut to Luka and Sam’s apartment. It’s dark. Luka calls their names. No one’s there. There are boxes everywhere. A note says, “She’s already started packing. One of the boxes is labelled, ‘Alex’s room’.” He gets a beer and finds the bottle opener near one of Alex’s action toy figures. It’s the one he stepped on in the teaser. He puts it in the box full of Alex’s things and…END OF EPISODE. Êîíåö
